The Community Advisory Program for the Elderly, known as CAPE, is an Outpatient Mental Health Clinic for persons aged 50 and over and their families. CAPE offers individual, group and family therapy, psychiatric evaluations and medication monitoring for both clinic and homebound older adults. Children of elderly parents, and family members or caregivers of persons with dementia may attend family treatment and caregiver support groups.

The Mobile Outreach Service Team (MOST) is available as part of CAPE’s services. The interdisciplinary team of social worker, registered nurse and psychiatric nurse practitioner, work to identify homebound elderly, assess and evaluate their psychosocial and health needs, and link the individual and family to the appropriate community programs. Services include ongoing supportive counseling and psychotropic medication monitoring.

CAPE provides onsite social work and psychiatric services at several satellite clinics throughout the borough of Queens. We have satellite clinics for adults ages 60 and above at the following locations:

  • HANAC Harmony ISC (Astoria – Spanish/English)
  • Peter Cardella Senior Citizens Center (Ridgewood – English)
  • Queens Community House: Rego Park Senior Center (Rego Park – Mandarin/English)
  • Selfhelp Clearview Senior Center (Bayside – English) – serves adults ages 50 and older
  • Self Help Innovative Senior Center (Flushing – Mandarin/English)
  • Sunnyside Community Services (Sunnyside – Spanish/English)
  • Theodora G. Jackson Adult Center (Jamaica – Spanish/Mandarin/English)

CAPE also provides telephone reassurance services to homebound or socially isolated older adults.

Transportation (daytime only), including a wheelchair accessible van is available for a small fee, for those unable to use public transportation.

Community Mental Health Services

Oasis at Commonpoint

Together with Northwell Health, Commonpoint Queens is proud to introduce to you a partnership program called “OASIS at Commonpoint Queens”, a one-time scheduled appointment that offers older adults, and their caregivers, a comprehensive cognitive and medical assessment. The practitioners providing the assessments are specialized in geriatric care and are trained to recognize geriatric syndromes, such as dementia, which can be easily overlooked or misunderstood in other clinical settings.

The OASIS assessment strategy is to connect patients to the services needed in order to maximize independence, days at home, and quality of life. OASIS is unique because it addresses the “whole person” in order to connect the patient and family with appropriate resources for their individual needs.
