Case Manager for Opportunity Youth (Elmhurst, Queens)

Commonpoint Queens is seeking to hire a Case Manager for Opportunity Youth. The goal of the Opportunity Youth Programs at Commonpoint Queens is to provide a career pathway for opportunity youth through a continuum of services including literacy and math instruction, HSE test preparation, work readiness training, college and career exploration, paid work experience, advanced occupational training and industry credential attainment. The programs support opportunity youth at any educational level, providing on-ramps at various levels through three program components, each supported by case management and wrap-around services.

Responsibilities:

  • Manage a caseload of 25-30 participants for the Advance and Earn program
  • Conduct intake Assessments and assist with eligibility documentation collection
  • Complete all required paperwork and documentation related to participants’ enrollment and completion of program
  • Provide participants with individual and group mental health support
  • Monitor participant program performance and  attendance  (i.e., employment upgrades, work-readiness training, advanced training, work site visits)
  • Provide detailed case notes and data entry on participants
  • Assist in the development and implementation of Individual Services Strategy for each participant
  • Identify and coordinate supportive services for participants, including referrals to community-based programs to remove barriers from participation and/or achieving success in the program
  • Provide crisis intervention when necessary, appropriate referrals, and home visits when needed.
  • Maintain effective communication and rapport with participants, staff, and support systems.
  • Conduct care coordination with outside treatment providers/referrals with the goal of improving participants well being and progress
  • Collaborate with support staff to address the individual needs of participants.

Qualifications:

  •  Master’s degree in human services or a related field required.
  • 3+ years of case management experience with opportunity youth
  • Demonstrated ability in youth development
  • Demonstrate cultural competency through familiarity in working with people from a very diverse number of backgrounds.

Histotechnician at Dermatology Group (Manhattan)

Responsibilities:

Homeless Outreach Specialist (NYC)
Hours: 37.5 hours per week, various morning, evening and overnight shifts

Jointly funded by the Department of Homeless Services and the Metropolitan Transportation Authority, BRC’s Transit Homeless Outreach staff members operate throughout the transit system 24 hours a day. Through this program, which BRC has operated since 2005 and significantly expanded in 2014, BRC provides outreach and case management services to humanely assist homeless individuals to permanently relocate from MTA properties. The program serves the New York City metropolitan area, including the boroughs of Manhattan, The Bronx, Brooklyn and Queens. Clients of Transit Homeless Outreach are engaged and assessed with the goal of connecting them with housing and essential services.

Responsibilities:

  • Responsible for providing direct outreach, escort, and referral services for all homeless individuals
    encountered in the field during their shift.
  • Recording and maintaining accurate statistical data on all individuals approached and referred as well as case-record management of clients receiving longer-term services.

  • Develop lesson plans and adjust instruction according to the various learning styles of program participants. Maintain a collaborative learning environment.

Responsibilities & Qualifications:

    • Assists with coordinating patient and family relations programs to strengthen partnerships with patients, families, staff and community. 
    • Promote patient and family centered care throughout the hospital. 
    • Responds to complaints and concerns and facilitates problem resolution. 
    • Tracks and maintains complaint database and prepares reports for leadership and staff.
    • Utilizes feedback to facilitate system improvements to better meet patient and
    • family needs. 
    • Provides appropriate emotional support (i.e. Bereavement support)
    • Interprets the Patient’s Bill of Rights and other policies and procedures of the hospital

MAINTENANCE

  1. Full Time Porter  Locations: Queens, Little Neck and Far Rockaway

Pay: $15 per hour

Responsibilities & Qualifications

  • Act as a Porter at multiple Commonpoint Queens locations, as required.
  • Routine cleaning of the buildings, including sweeping, mopping, collecting garbage, cleaning of classrooms, locker rooms and bathrooms, etc., and other tasks associated with maintaining the building in a manner expected of a first-rate membership-driven facility.
  • Room set-ups as the schedule requires, and to be available for “as needed” cleaning where necessary and working in the building “whenever needed.”
  • Work toward obtaining a Commercial Driver’s License, in order to provide transportation to all populations on an as-needed basis.
  • Experience providing high quality services as a porter for three (3) or more years.
  • Ability to perform physical activities such as climbing, lifting, balancing, walking, stooping, and handling of materials.
  • Knowledge of the proper use of household tools and cleaners.
  • High School Required
  • 3-5 years of experience preferably in a not for profit organization
  • Possess outstanding social skills and emotional maturity.

  • New York State security guard license is required.

Pay $17 per hour 

Responsibilities & Qualifications:

    • Responsible for ensuring the safety and security of all client’s, staff, and the property around the facility in a homeless shelter setting.  The populations served are single men and single women.  
    • High school diploma or GED and prior security experience required. 
  • Must have NYS Security Guard License, and current 8 Hour Annual Certificate. 
  • Fire Guard License (F02).
  • Certified in Nonviolent Crisis Intervention (CPI) preferred.
  • Must be able to work flexible shifts, weekends and overtime if necessary.

RETAIL
